Workplace Approval Form

I, the abovenamed supervisor, allow Applied Vocational Training students to undertake a placement with this facility under my supervision.

I will assist the student in the development and application of the required skills and knowledge needed in a practical animal workplace setting and understand that the student is required to provide evidence for assessment in the form of videos, photographs and workplace documentation.

I confirm, the abovenamed business offers an environment that is non-discriminatory and free from any type of bias, harassment, prejudice or bullying.

I agree to provide both verbal and written feedback on the student’s performance to Applied Vocational Training at regular intervals throughout the duration of the student’s placement.
