Please use this form to request a Reissue of a Certificate (Testamur), Record of Results or Statement of Attainment. Please note, there is a $50 fee for reissuing a qualification document.
Once we have processed your submission and confirmed your enrolment with us, we will issue an invoice for the total amount using the contact details provided. Once this invoice has been paid, we will reissue the qualification and either post the document or notify you when it is ready for collection (depending on the option chosen below).
